When a young person moves to another area, they need to be transferred to a the Youth Service provider with the contract for that area.

When a young person tells you that they have moved to another area, you must send a notification to YSSU who will confirm the transfer out of your service.

Prior to the young person transferring out of your service, you will need to result all activity participation against the young person’s profile in ART and send a notification to YSSU with:

  • the young person’s new address, and
  • any supporting information you hold about the young person relevant for the next Provider is captured in ART.

If there are any safety concerns, you must notify the new provider. If the young person has accommodation redirections in place, you will need to submit a task to YSSU to have these cancelled.

All critical documents should have already been scanned and uploaded into ART. Once the transfer has been accepted, the new provider will have access and visibility to the young persons information in ART.

YSSU will approve the transfer of the young person to the new provider who will make contact with the young person to make a follow up appointment.

If the young person makes contact with a new provider first, the new provider must send a notification to YSSU confirming:

  • the young person’s new address (note: the young person can also do this online through MyMSD)
  • their accommodation costs
  • any other changes to their circumstances.

YSSU will add notes into ART and send a notification to the young person’s old provider to complete the transfer process.
