We’ve developed a consent form to support providers who want to use photos and videos of young people enrolled in Youth Service in their publications.

who can consent

The form allows Youth Payment and Young Parent Payment (16 to 17 years) to grant consent autonomously, permits young parents to give consent for a child or person under 14 years old in their care, and enables NEET young people aged 15 years to consent.

completing the consent process

To complete the consent process, you will need to assess if the young person understands what they are consenting to. This assessment should be documented in the 'Office use only' section of the form.

upload the form to art

Once consent is acquired and the form is fully completed, signed and dated, it must be scanned and uploaded to the young person's ART record.

the form is locked

The form is locked to avoid any modification beyond its intended use. The provider can still add their logo into the header. Text can also be added into the editable fields.

be mindful

It is advised not to take photos and videos of young people holding their personal information, e.g., achieved licence.  Someone may use their details from their licence for fraud purposes.
