The initial Youth Service plan helps identify what interventions are required to address any barriers for your young person to participate in education, training, work-based learning, or employment (for example, mental health issues, drug, and alcohol counselling).

You will complete a Youth Service plan which must detail:

  • A young person's circumstances.
  • Activities and goals agreed by both of you.
  • Other interventions or supports that are needed to assist the young person towards independence.
  • The commitment the young person has agreed to.
  • How you are going to support the young person to achieve this.

The plan is non-specific, and you can tailor it to support the young person’s needs, taking into consideration their personal circumstances.

Your service may have your own assessment process and tools, or you can use the Youth Service plans below to capture the minimum standard of information about a young person.  These plans can be prefilled and printed and completed by you or the young person.  The information recorded should prompt discussion between you and the young person.  A copy of the Youth Service plan should be given to the young person.

All information from the plan and the plan itself must be signed by the young person and uploaded in ART. This is a contract requirement. You must complete a Youth Service plan within 30 days of enrolling the young person in ART and it must be reviewed at least every 90 days with face to face contact.  The young person’s Youth Service plan should also be reviewed to reflect any new activities.
