The NEET consent form is used to get a young person’s permission to participate and enrol into the NEET programme.

what does it do

It provides consent to the information sharing that underpins the NEET service, such as sharing information between us (the Ministry), you (the Provider), schools and other agencies.  It includes receiving wraparound, intensive support, to help them get on track for study, training, work-based learning or employment.

what its used for

The NEET consent form is used for all NEET enrolments into the Youth Service NEET programme.

the form is locked

The form is locked to avoid any modification beyond its intended use. You can still add your logo into the header.  Text like your provider name can also be added into the editable form. A printable form is also available.

Once consent is acquired and the form is fully completed, signed and dated, it must be scanned and uploaded to the young person's ART record.

other activities

The NEET consent form must not be recreated to include other activities. If a provider requires consent for other activities not covered in this form, they must create a separate consent form for those "specific" activities.

A provider wants to contact and engage with creditors (like a landlord) on a young person's behalf. The provider wants to include this into the NEET consent form.

The provider cannot alter or recreate the NEET consent form to include other activities.

The provider can create a separate consent form for other activities instead.
